If you are a new patient, this information will be given to you when you call the office to schedule and register. For our existing patients, you will be given a letter containing this code.

To start, please click the icon in the bottom right corner for the patient portal. Then, click on the link Activate patient portal account.

You will be prompted to fill out some basic personal information, as well as create a login name and password. Please enter your name and date of birth exactly as it appears at the top of this letter. You will be able to make changes to your personal information once your portal account has been activated.

Please be as thorough as possible when filling out your personal information and medical history. This will help us provide you with the best medical care possible.
